胃カメラの話題ですが、一部全身麻酔を使うと監視外されていらっしゃる方が多いでになるご様子です。その件について一言コメントと私個人の見解を書かせてください。
全身麻酔では自発呼吸を止めます。
そうなんだ。
鎮静法では自発呼吸を止めません。これが大きな違いです。
一般のクリニックで行う検査で呼吸を止められてしまうのはそれこそ怖くないですか?
胃カメラ検査で使うのは鎮静法ですから、自分で呼吸ができる程度の意識レベルを維持します。
鎮静剤は体重1kgあたりの薬材量が決まっているので、単純に体重から計算された鎮静剤の量を整柱し、検査が行われることがあります。
この時、鎮静剤に対する個人の感受性の違いまで考えてもらえないことがあり、これが快適に検査を受けられるかどうかの大きなポイントでしょう。
つまり、鎮静剤が効きにくい人は鎮静震度が浅い状態のまま検査になることがあるということです。
私は築地の大きな病院で胃カメラを鎮静科でやってもらった際、効きが悪くとても辛い思いをしたことがあります。
この不安をできるだけ回避するために少し費用はかかりますが、検査専門のクリニックさんで人間ドッグの項目の一つとして胃カメラを行ってもらうと丁寧に見てもらえるはずです。
クリニックでも上手に鎮静をして上手に検査してくれる施設はありますが、一般に診察と検査が同時進行している忙しいクリニックさんは個々の鎮静震度まではあまり気にしてくださらないかもしれません。
